Maybe it’s not Safari, as I must say I used to have this problem with Internet Explorer. In any case, there’s only one sure way I’ve ever been able to force a refresh (without the whole foolishness of emptying the cache etc): force a refresh with iCab.

Now, I don’t really use iCab for anything else. Although it’s a lovely browser with all kinds of nifty features it just doesn’t handle Style Sheets very well. But it sure can Refresh. Once I’ve forced a Refresh with iCab I can go back to Safari and force a refresh there.

It could all be something to do with my Internet connection: cable modem into an iMac then wireless to my Powerbook. In any case, I don’t know why this happens, but I do know iCab works. If you have any thoughts then feel free to leave a comment.

Your ISP is running a transparent web cache. Safari currently fails to send the Refesh: command to the web cache, thus expired pages that are not past their expiration date are not flushed. IE used to have a problem with an IMS-REFRESH request, which was fixed in versions 5.5SP1. Until Apple fixes Safari, you’re going to have to continue using your workaround.
